DVA Form D2582A, the Additional Information Sheet, is a companion document to Form D2582, the Claim for Disability Compensation Payment. It is not a claim form in its own right. It gives the veteran space to describe additional conditions being claimed, and it gives the treating medical practitioner a structured place to record a diagnosis for each one, together with the basis for that diagnosis. The completed sheet is attached to the D2582 claim before lodgement.

What is DVA Form D2582A used for
The main D2582 claim form has room for a limited number of conditions. The Additional Information Sheet extends it, carrying further conditions in the same two column format: the veteran describes each disability, and the medical practitioner records a diagnosis next to it. DVA prefers a final diagnosis but accepts a provisional one where that is all the doctor can give.
The sheet covers new conditions only. Injuries or diseases that DVA has already accepted as war caused or defence caused should not be listed again. For each condition, the veteran also explains how they believe their service caused, contributed to or aggravated it, and roughly when the signs and symptoms first appeared.
Who completes DVA Form D2582A
Two people complete the form. The veteran fills in the disability descriptions, listing each condition and its signs and symptoms as fully as possible, for example pain in the lower back or loss of range of movement in an arm, even if the formal diagnosis is unknown.
The treating medical practitioner then completes the medical column for each condition: the diagnosis, whether it is final or provisional, the basis for it, and when the veteran first consulted them about it. The doctor attaches any confirming reports, applies their stamp or practice details and signs the sheet. DVA pays the practitioner for this service according to its schedule of fees once an account is lodged.
How to fill out DVA Form D2582A
Complete your sections before the medical appointment so your doctor can focus on the diagnosis fields.
- List each new condition you are claiming, one per numbered disability block.
- Describe the signs and symptoms in plain language if you do not know the diagnosis.
- Explain how you believe your service caused, contributed to or aggravated each condition.
- Record the approximate date you first became aware of the signs and symptoms.
- Ask your doctor to complete the diagnosis, basis for diagnosis and consultation date fields for every condition, attach any confirming reports, and sign and stamp the form.
- Attach the completed sheet to your D2582 claim. If you are claiming more conditions than the sheet holds, attach a separate page with the same details.
Lodging the sheet with your claim
The Additional Information Sheet must travel with the D2582 claim form. Lodging it separately, or lodging the claim without the diagnosis information, is likely to slow the assessment because DVA will need to come back to you or your doctor for the missing detail.
