What is DVA Form D0507 used for

Qualifying service is the legal threshold that determines whether a veteran can receive a service pension. For Commonwealth and Allied veterans, DVA needs detail about where, when and with whose forces the person served, whether they served outside the country of enlistment, what campaign stars or medals they were awarded, and whether they directly experienced danger from hostile enemy forces.

The form also covers specific situations, including service with certain New Zealand Defence Force units integrated with Australian forces during the Vietnam War, and individual NZDF members attached to Australian units. Veterans who served with the armed forces of the Republic of Vietnam, or with forces in Yugoslavia during the Second World War, must complete the additional attachment forms the D0507 refers to.

Who uses DVA Form D0507

Veterans of Commonwealth or Allied forces who are considering a service pension use this form to have the qualifying service question decided first. The form asks whether you lived in Australia for the 12 months immediately before joining the relevant service, and whether you ever served with or assisted enemy forces, both of which affect eligibility.

The form carries a practical warning worth taking seriously: DVA accepts no responsibility for actions such as early retirement taken in anticipation of a favourable decision. Even with qualifying service determined, the service pension still depends on other requirements such as the income and assets tests and residency, so DVA advises having eligibility confirmed in writing before making retirement decisions.

How to fill out DVA Form D0507

Write in block letters with a blue or black pen, or type the entries on screen. Answer every question unless the form directs otherwise.

• Complete your personal details, including your DVA file number if you have one and your place of birth.
• Record your service number and any other names you served under, attaching certified copies of name change documents.
• State which country's forces you served with, and complete the additional attachment forms if you served with the armed forces of the Republic of Vietnam or with forces in Yugoslavia during the Second World War.
• List each period of service separately, with branch, place and date of enlistment and discharge, using approximate dates if you are unsure.
• Detail service outside the country of enlistment, campaign stars and medals with documentary evidence, and the nature of your duties.
• If applicable, describe specific incidents where you directly experienced danger from hostile enemy forces, with dates, places and your role at the time.
• Sign the declaration and attach certified copies of discharge certificates, statements of service or medal citations, with certified English translations for any foreign language documents.

After the determination

DVA may seek information from other organisations to verify the service you describe, and the declaration authorises that exchange. Once qualifying service is determined, you will be advised in writing.

A favourable determination does not itself pay anything. To claim the service pension you then lodge the service pension claim forms, and the income and assets, age or invalidity, and residency requirements all still apply.
