DVA Form D0648 is the Claim for Service Pension or Income Support Supplement Part B, Income and Assets. It is the financial half of the two part paper claim: Part A, lodged as Form D0503 for veterans or D0504 for partners, widows and widowers, establishes eligibility, while Part B sets out everything DVA needs to apply the income and assets tests. The two parts should be lodged together so the claim can be assessed without delay.

What is DVA Form D0648 used for

Service pension and income support supplement are means tested payments. Form D0648 collects the detail of your financial position: bank, building society and credit union accounts, cash holdings, shares and other securities, bonds and debentures, money on loan, managed investments, superannuation in accumulation phase, defined benefit and other income streams, self managed superannuation funds, gifts made in the last five years, employment income, overseas payments, money from boarders or lodgers, and any other income.

If you have a partner, their income and assets must be declared as well, even if the partner is not claiming or already receives a DVA income support payment. Each item is marked as owned by you, your partner or both.

Who uses DVA Form D0648

Anyone lodging a paper claim for service pension or income support supplement completes this form as Part B. That includes veterans claiming through Form D0503 and partners, former partners, widows and widowers claiming through Form D0504.

If your finances are complex, the form notes you may choose to engage an accountant or financial adviser at your own expense to help you complete it, but you, and your partner where applicable, must sign it yourselves. Ex-service organisation advocates are also experienced with these claims.

How to fill out DVA Form D0648

Gather your latest statements before you start, because most questions require certified copies of supporting documents.

  • List every bank, building society and credit union account, including term deposits and joint accounts, together with the account your pension would be paid into.
  • Declare cash holdings above the threshold stated on the form, shares, bonds, debentures and money you have lent to anyone, including family members.
  • Detail managed investments, superannuation funds still in accumulation phase, defined benefit pensions and any other income streams, attaching the latest statement or income stream schedule for each.
  • Disclose self managed superannuation fund or small APRA fund membership with the fund's tax return, member statement and property records where relevant.
  • Record gifts, assets sold for less than their value, or surrendered rights from the last five years.
  • Report current employment income, payments from overseas agencies, money from boarders or lodgers, and any other income not covered elsewhere.
  • Tick the You or Partner box, or both, for every item, and sign the form along with your partner where required.

Supporting documents and lodgement

Almost every Yes answer on the form triggers an attachment: certified copies of share statements, loan documents, superannuation statements, income stream schedules and overseas payment advices. Foreign documents must be in English or accompanied by an accredited translation. Providing complete documents up front is the single biggest factor in how quickly the claim can be assessed.

Lodge Part B together with the matching Part A form. If your circumstances change after lodgement, tell DVA, since income support payments are adjusted to reflect current income and assets. Questions about how a particular investment is assessed are best put to DVA directly or raised with an ex-service organisation advocate.
