Form SA369 is the Services Australia income and assets form. It tells Centrelink about your income and assets, and your partner's, so a payment can be worked out or reassessed. It arrives inside claim packs such as the Age Pension claim and is also used on its own when Services Australia asks for updated financial details.

What is Form SA369 used for
Most Centrelink income support payments are means tested, and SA369 is the standard paper vehicle for the financial details behind that test. It covers employment income, self employment, real estate, bank accounts, shares and investments, superannuation, income stream products, money on loan, gifts, and interests in private trusts and companies.
Some answers point you to companion modules for the finer detail, such as Real estate details (Mod R) for property, Private Trust (Mod PT) for trust interests, or a separate Partner details form. If you are making a claim, the form must go back with all your other supporting documents at the same time, or the claim may not be accepted.
Who uses Form SA369
Anyone claiming or receiving a means tested Centrelink payment can be asked to complete it, most commonly alongside claims such as Age Pension. If you have a partner, you must include their income and assets as well; couples can complete one form together or a separate SA369 each.
Many customers update the same details through their Centrelink online account via myGov instead, which Services Australia describes as the easier route. The paper form remains the option for claims lodged on paper and for people who prefer it.
How to fill out Form SA369
Gather recent statements before you start, because the form asks for current balances and values:
- Report employment income such as wages and salaries at gross amounts, before tax.
- Declare self employment or business involvement, which may require a Profit and Loss Statement or business financials.
- List bank, savings, and cheque accounts with current balances, plus shares and other investments.
- Detail superannuation you hold where the fund is not paying a pension, and any income stream products such as account based pensions or annuities.
- Disclose real estate other than your home, money you have lent, and gifts or transfers of assets, including transfers involving trusts or private companies.
- Complete any companion modules the form points you to, then sign and date the declaration. An accountant or financial adviser can help prepare the form, but you must sign it yourself.
How to submit Form SA369
Return the form and supporting documents online through your Centrelink online account (identity documents excluded), or lodge them in person at a service centre. If the form came inside a claim pack, follow the lodgement instructions for that claim and send everything together, since claims are generally not processed until all documents arrive.
Forms are revision dated and Services Australia can reject superseded versions, so download the current SA369 from servicesaustralia.gov.au before lodging.
