Form SU580 is the Services Australia Profit and Loss Statement. Sole traders, including subcontractors, and partners in a partnership use it to report business income and expenses for a set period when they have started new employment or a new business, or when their level of business activity or income from self employment has changed. Centrelink uses the figures to assess income support payments.

What is Form SU580 used for
The statement gives Centrelink a current picture of business profitability when past financial statements no longer reflect reality. It records gross business income for the period, splits expenses into non variable items, such as insurance premiums, interest on business borrowings, licence fees, and depreciation claimable under Division 40 of the Income Tax Assessment Act 1997, and variable items, such as advertising, materials, motor vehicle running costs, and wages paid, then arrives at a net profit or loss.
Do not use SU580 if your previous year's financial statements are still indicative of current profitability, if you are a wage or salary earner or pieceworker, if Services Australia has advised you are in an employee style relationship and must declare gross income, or if your business operates through a private company or trust.
Who completes Form SU580
Sole traders complete it for their own business. Partners in a partnership complete it as well, recording the number of business partners and each partner's percentage share of income. If you are neither a sole trader nor in a partnership, the form directs you to call Services Australia on 132 850 to confirm whether it is needed at all.
Centrelink also asks for a copy of your latest personal income tax return, the partnership return where applicable, and supporting records such as a balance sheet, profit and loss statement, and depreciation schedule, unless you have already provided them.
How to fill out Form SU580
The form is short, but the financial page rewards preparation. Have your records for the period on hand, then:
- Enter your Customer Reference Number, name, date of birth, business name, business type, and contact number.
- Answer the sole trader and partnership questions, including income shares if you have partners.
- Confirm whether your latest tax returns and supporting documents have already been provided.
- Set the period of the statement with from and to dates.
- Complete the financial details page: gross income, non variable expenses, variable expenses scaled to the statement period, and the resulting net profit or loss.
- Date the declaration. Sign by hand only if you are returning the form by post.
How to submit it
Return the form and copies of supporting documents within 14 days of making a claim or of the change in circumstances. If you cannot meet that deadline, contact Services Australia as early as possible to make an arrangement.
You can upload the saved form and documents through your Centrelink online account in myGov, which needs no signature, or post them to Services Australia, Centrelink, Reply Paid 7800, Canberra BC ACT 2610 with a handwritten signature on the declaration.
