Form SA002 is the Services Australia paper claim for Age Pension and the Pension Bonus. Age Pension provides income support and access to concessions for eligible older Australians, while the Pension Bonus is a lump sum for people already registered in the closed Pension Bonus Scheme who deferred claiming Age Pension while they kept working. Services Australia treats the online claim through a Centrelink online account as the faster route, so the paper pack is the alternative for people who cannot or prefer not to claim online.

What is Form SA002 used for
SA002 collects the personal, relationship, residence, and payment details Services Australia needs to assess an Age Pension claim. The claim pack pairs it with the Income and Assets form (SA369), and depending on your answers you may also need companion forms such as Partner details (Mod P) or Real estate details (Mod R).
The same claim covers the Pension Bonus for people registered in the Pension Bonus Scheme. The scheme is closed to new registrations, and registered members must claim the bonus at the same time they claim Age Pension.
Who uses Form SA002
People claiming Age Pension on paper use SA002, and claims can be lodged up to 13 weeks before reaching Age Pension age. Couples where both partners are claiming can complete one claim pack together or a separate pack each, and a partner who is not claiming still needs to provide their details, either on this form or on a separate Mod P form.
People who are permanently blind follow an extra step: the pack refers them to a Request for Ophthalmologist/Optometrist Report (SA013) completed by their treating specialist. People receiving a Disability Compensation Payment from the Department of Veterans' Affairs can choose to have their Age Pension paid by DVA and lodge their forms there instead.
How to fill out Form SA002
Read the Information Booklet (Ci006) alongside the claim, then work through the pack in order:
- Answer the partner questions first, since they determine whether your partner claims on the same form, a separate SA002, or provides details on a Mod P form.
- Enter your Customer Reference Number if you have one, along with your name, date of birth, and contact details.
- Complete the residence questions covering your Australian residence history.
- Answer the Pension Bonus section only if you are a registered member of the closed scheme claiming the bonus with your pension.
- Complete the Income and Assets form (SA369) that comes in the pack. An accountant or adviser can help prepare it, but you must sign it yourself.
- Use the checklist to confirm the identity documents and supporting papers, then sign and date the declaration.
How to submit Form SA002
Lodge the claim online by uploading it through your Centrelink online account (identity documents excluded), post it to Services Australia, Seniors Services, PO Box 7808, Canberra BC ACT 2610, or hand it in at a service centre. Lodging as soon as possible protects your start date, and you can lodge the claim form first and follow up with outstanding documents, although the claim cannot be processed until everything is in.
Forms are revision dated and Services Australia can reject superseded versions, so confirm you have the current SA002 on servicesaustralia.gov.au before lodging.
