DVA Form D2582 is the Claim for Disability Compensation Payment and/or Application for Increase in Disability Compensation Payment, published by the Department of Veterans' Affairs. Veterans, including merchant mariners, use it to claim compensation for conditions they believe are related to their service under the Veterans' Entitlements Act 1986, or to apply for an increase where an already accepted condition has worsened. Another person can lodge the claim on a veteran's behalf. Most claims are now lodged online through DVA's MyService portal, and the paper form remains the alternative for people who prefer it or cannot use the online channel.

What is DVA Form D2582 used for

The form serves two purposes. A veteran who has not previously had a condition accepted as war caused or defence caused uses it to make a new claim for a Disability Compensation Payment. A veteran whose conditions are already accepted uses the same form to apply for an increase in the payment where those conditions have deteriorated.

The claim is assessed under the Veterans' Entitlements Act 1986. Decisions about whether a condition is service related are based on the Repatriation Medical Authority's Statements of Principles, which set out the medical and scientific factors that connect particular conditions to service. Members of the Australian Defence Force with service on or after 1 July 2004 may fall under the Military Rehabilitation and Compensation Act 2004 instead, and the form's instructions point those claimants to a different claim form, so it is worth confirming which Act covers your service before you start.

Who uses DVA Form D2582

The form is completed by veterans with service covered by the VEA, or by someone acting on a veteran's behalf. Part A lets you nominate a representative, such as an ex-service organisation, a legal practitioner or another person, to act for you in matters connected with the claim.

DVA strongly encourages claimants to get help from an ex-service organisation of their choice. Trained advocates, including those accredited under the Advocacy Training and Development Program, can explain how the Statements of Principles may apply to your conditions and help you assemble supporting documents. DVA staff can also assist with completing the form itself.

How to fill out DVA Form D2582

The form runs to fifteen pages and asks for detailed personal, service and medical information. Having each condition properly diagnosed before you complete the form helps prevent processing delays.

  • Decide whether to nominate a representative in Part A, and record their details if so.
  • Complete your personal details in Part B, including your DVA file number if you have one, your contact details and relationship status.
  • List each condition you are claiming and describe how you believe it relates to your service. A written statement or contention explaining the connection strengthens the claim.
  • Attach supporting documents where you have them, such as your service record, ADF medical documents, specialist reports, scans and any incident reports. If you no longer have access, DVA can request records from the ADF on your behalf.
  • Check the proof of identity requirements. Depending on when you joined or separated from the ADF, identity may be established automatically, otherwise certified copies of identity documents may be requested.
  • Sign the authorisation and declaration before lodging, and keep a copy of everything you send.

Lodging the claim and what happens next

If you have a PMKeyS number, DVA suggests considering MyService, its online claim portal, which guides you through the same information interactively. Paper claims are posted to the Department of Veterans' Affairs at its Brisbane GPO address printed on the form, and DVA can be contacted on its general enquiries line for help at any stage.

After lodgement, DVA assesses the claim against the Statements of Principles for each claimed condition, or against the best available medical and scientific evidence where no Statement of Principles exists. You may be asked to attend medical examinations or provide further documents. If any details you gave in the form change, you must tell DVA within 21 days. Giving false or misleading information is a serious offence.
