DVA Form D0800 is the Claim for Travelling Expenses, published by the Department of Veterans' Affairs. It reimburses travel connected with treatment, a disability claim or an income support claim under the Veterans' Entitlements Act 1986, the Military Rehabilitation and Compensation Act 2004, the Safety, Rehabilitation and Compensation (Defence-related Claims) Act 1988 and the Australian Participants in British Nuclear Tests (Treatment) Act 2006. It is a short two page form covering two trips, with space for vehicle, taxi, public transport, air, tolls, parking and accommodation costs.

What is DVA Form D0800 used for
The form claims reimbursement for travel to approved health providers and for travel related to disability or income support claims, including hospital admissions and discharges. For each trip you record the date, the provider attended, the purpose of the trip and what you are claiming for the return journey, whether kilometres in a private vehicle or amounts for taxis, public transport, air travel, road tolls and parking.
It is not the right form for every kind of DVA related travel. Travel connected with the Administrative Appeals Tribunal, the Veterans' Review Board or the Specialist Medical Review Council is claimed on a different form, and DVA state offices or Veterans' Affairs Network offices can point you to it.
Who uses DVA Form D0800
Veterans and other DVA clients who travel for approved treatment or in connection with a claim complete the form themselves, and a person authorised to act on the claimant's behalf can complete and sign it instead, providing their own name and address in the space provided.
To receive the maximum entitlement, travel should generally be to the closest practical provider. If DVA arranged or paid for the travel itself, for example a booked car with driver or air travel, that is declared on the form so the same costs are not claimed twice.
How to fill out DVA Form D0800
Complete all relevant questions in the claimant sections. An incomplete or incorrect form may be returned to you, which delays payment.
- Enter your name, DVA file number, contact details and address, or the details of the authorised person acting for you.
- For each trip, record the date and time, the type and name of the health provider, their suburb and phone number.
- Tick the purpose of the trip: treatment, disability claim, income support, hospital admission or discharge, or another purpose you specify, and note what you were treated for.
- Set out what you are claiming for the return trip: kilometres for a private vehicle, or amounts for public transport, taxi, air travel, road tolls and parking.
- Declare whether DVA arranged or paid for any of the travel, whether you needed accommodation and its type and length, and whether you travelled with a medically required attendant.
- Sign and date the declaration, then post the form to DVA's Brisbane GPO address printed on the form.
Time limits and receipts
The form must be lodged within 12 months after the travel is completed. Keep receipts for larger expenses for four months after claiming, because DVA may ask to see them during that period. The relevant DVA factsheet on travel for treatment explains the current allowances and conditions.
