The Medicare claim form (MS014) is the Services Australia form for claiming a Medicare benefit by mail. Most claims today are lodged electronically at the doctor's practice or through a Medicare online account, so MS014 exists for the cases those channels do not cover: paid or unpaid accounts a patient claims themselves, claims an individual lodges on behalf of a patient, and claims lodged by a business or third party.

What is Form MS014 used for
MS014 claims Medicare benefits for medical or dental services when you claim by mail. It covers both paid and unpaid accounts. The claimant is whoever paid, or is liable to pay, the expense, and that is who the benefit is paid to, whether that is the patient, another individual, or a business or third party such as an executor acting for a deceased claimant's estate.
All Medicare benefit payments are made by electronic funds transfer, so the form collects the bank, building society, or credit union account details the payment should go to.
Who uses Form MS014
Three groups use the form: patients claiming on their own behalf, individuals claiming on behalf of a patient, and businesses or third parties claiming on behalf of a patient. The form routes each group to different questions, so identify your situation at question 2 before you start.
In practice, MS014 is the fallback when a practice does not lodge the claim electronically, when an online claim is not available for your situation, or when someone is finalising claims for another person, such as the executor of an estate.
How to fill out Form MS014
The form is short, but the supporting documents matter as much as the answers:
- Enter the patient's Medicare card number and reference number exactly as they appear on the card.
- Tick whether you are the patient, an individual claiming for a patient, or a business or third party, and follow the question the form points you to.
- Provide the claimant's contact details and postal address.
- Complete the bank account section, including BSB, account number, and the name the account is held in. Accounts with restrictions on EFT deposits cannot receive the payment.
- Read the claimant's declaration carefully. It confirms you paid or are liable for the expenses and that the services are not excluded categories such as health screening or employment related services.
- Print the completed form and sign and date it by hand. The declaration cannot be signed digitally.
How to submit Form MS014
Post the signed form with the original invoices for the services you are claiming, plus proof of payment if you paid in full, to Services Australia, Medicare, GPO Box 9822 in your capital city. This form is only for claims lodged by mail; if you can claim at the practice or through your Medicare online account, you do not need MS014.
Confirm you are using the current revision of the form on servicesaustralia.gov.au before posting, since superseded versions can be rejected.
