What is Form MS004 used for

MS004 handles several enrolment situations in one document. Part A covers enrolling in Medicare for the first time, re-enrolling, or extending Medicare eligibility, including returning residents and visitors from countries that have a Reciprocal Health Care Agreement with Australia. Part C covers enrolling a newborn child born in Australia, and Part D covers My Health Record questions for the people being enrolled.

The form also collects bank account details, because Services Australia pays Medicare benefits by electronic funds transfer, and it lets you request a Lifetime Health Cover letter as proof of your Medicare registration date for a private health insurer.

Who uses Form MS004

The form is used by Australian citizens, New Zealand citizens residing in Australia, permanent residents, people who have applied for permanent residency or a permanent protection visa, visitors from Reciprocal Health Care Agreement countries, and certain other visa holders covered by Ministerial Order. Each category has its own document checklist inside the form.

Parents also use it to enrol a newborn child born in Australia, although a newborn can often be enrolled through a Medicare online account instead. Everyone listed on the form who has a different immigration status or visa entitlement end date needs a separate form, because they cannot share the same Medicare card.

How to fill out Form MS004

Work out which enrolment type applies at question 1 first, because it determines which parts of the form you complete:

• Select your enrolment type: first enrolment, re-enrolment or eligibility extension, or a newborn enrolment.
• Complete the contact person details. This person receives the Medicare cards and correspondence for everyone listed.
• Gather the documents for your category, such as a passport, birth certificate, visa evidence, or proof of permanent residency, and copy both the front and back of each one.
• Provide residency documents dated within the last 6 months where the checklist asks for them, for example proof of a lease, employment, or an Australian bank account.
• Answer the My Health Record questions in Part D for the people listed in Parts A and B.
• Add your bank account details so benefits can be paid, and sign the declaration.

How to submit Form MS004

The current revision of the form accepts lodgement by email or post. You can email the completed form and supporting documents to the address printed on the form, keeping attachments in an accepted format, not password protected, and within the size limits the form lists. Otherwise post it to Services Australia, Medicare, PO Box 7856, Canberra BC ACT 2610. Staff at a Medicare service centre can also help if you are unsure which documents you need.

Services Australia updates its forms regularly and can reject superseded revisions, so confirm you have the current version of MS004 on servicesaustralia.gov.au before you lodge it.
