What is Form MS011 used for

The form covers four requests. A transfer to a new card takes a person off their old card and issues them a card of their own, for example a teenager 15 or older leaving a parent's card. A copy to a new card gives the person their own card while keeping them active on the old one, such as a child at boarding school. A transfer to an existing card moves someone onto another card entirely, for example a couple joining one card, and a copy to an existing card keeps them active on both, such as a child copied onto a carer's card.

The form also collects bank account details, because Services Australia pays Medicare benefits by electronic funds transfer and holds benefits until it has current bank details. Note that MS011 does not change a Medicare Safety Net family registration; that is a separate form, MS016.

Who uses Form MS011

Typical users include teenagers and young adults getting their own Medicare card, parents arranging for children to appear on the cards of both households, couples consolidating onto one card, and carers copying a child in their care onto their card. Person 1 on the form must provide identification, such as an Australian passport, birth certificate, citizenship certificate, driver licence, ImmiCard, or a foreign passport with a valid Australian visa.

For a child younger than 15, the requesting parent or guardian provides the identification and completes the declaration on the child's behalf. If the child has not previously been enrolled with you on a Medicare card, you also need proof of parental responsibility, or proof the child is in your care if you are not the parent or guardian.

How to fill out Form MS011

Have the Medicare cards involved in front of you before you start:

• Tick one option for person 1: transfer to a new card, copy to a new card, transfer to an existing card, or copy to an existing card.
• Enter person 1's current Medicare card number and reference number, name, date of birth, and gender.
• Add the details of any additional people being copied or transferred. Anyone 15 or older fills in their own details and consents to their own declaration.
• Provide current contact and address details so the new or updated card is posted to the right place.
• Complete the bank account section so Medicare benefits can be paid.
• Sign the declaration for person 1, or as the parent, guardian, or primary carer if person 1 is younger than 15, and attach copies of the identification documents.

How to submit Form MS011

The current revision accepts lodgement by email or post. Email the form and supporting documents to the Medicare enrolment address printed on the form, keeping attachments in an accepted format, not password protected, and within the size limits listed, with the words copy or transfer in the subject line. Otherwise post it to Services Australia, Medicare, PO Box 7856, Canberra BC ACT 2610.

Services Australia revises its forms and can reject superseded versions, so download the current revision of MS011 from servicesaustralia.gov.au/ms011 before you lodge it.
