Who files Form DS-82
You can renew with DS-82 only if every one of the eligibility conditions holds. If any condition fails, you must apply in person with Form DS-11 instead.

How to fill out Form DS-82
The application mirrors the DS-11 but is shorter because your citizenship was already established. Type carefully; mismatches with your prior passport slow processing.

- Most recent passport number and issue date
- Name change information and the supporting document, if your name differs from the passport being renewed
- Product selection: passport book, passport card, or both

- Sign and date after printing; unlike the DS-11, you sign the DS-82 yourself before mailing
How to submit it
Mail the signed form, your most recent passport, one compliant passport photo stapled as instructed, any name change document, and the fee payment to the address listed in the form instructions for your state and service level.
