Form DS-11, Application for a U.S. Passport, is the U.S. Department of State form for anyone who cannot renew by mail: first-time applicants, children under 16, applicants aged 16 and 17, and people replacing a lost, stolen, or long-expired passport. Every DS-11 application is submitted in person.

What is Form DS-11 used for

The DS-11 is the standard application for a new U.S. passport book, passport card, or both. It is used whenever the mail-in renewal form DS-82 is not allowed, and it requires you to appear in person so an authorized agent can verify your identity and witness your oath and signature.

  • All passports for children under 16, who must apply in person with their parents or guardians
  • Applicants aged 16 and 17
  • Replacement of a lost or stolen passport, filed together with Form DS-64
  • Previous passport issued more than 15 years ago, issued before age 16, or damaged

How to fill out Form DS-11

The form collects identity and contact information plus details the State Department uses to verify citizenship. Federal law requires your Social Security number if you have one.

  • Name, date and place of birth, Social Security number, and contact information
  • Parents' names, dates of birth, and places of birth, used for citizenship verification
  • Product selection: passport book, passport card, or both, and regular or large book
  • Emergency contact and travel plans
  • Height, hair color, eye color, and occupation
  • Most recent passport details if you had one
  • Leave the signature line blank; you sign only when the acceptance agent tells you to

How to submit it

Bring the unsigned DS-11 to a passport acceptance facility, such as many post offices, clerks of court, and public libraries, or to a passport agency if you qualify for an urgent appointment. The acceptance agent reviews your documents, administers the oath, and watches you sign.

Bring proof of U.S. citizenship, such as a certified birth certificate, plus a photocopy of it, a valid photo ID plus a photocopy, one passport photo meeting the requirements, and payment. Fees vary by product and service speed, so check the current amounts on travel.state.gov before you go. For children under 16, both parents or guardians generally must appear, or the absent parent provides Form DS-3053.

Frequently asked questions

Your signature must be witnessed. The acceptance agent administers an oath and watches you sign, which is part of how the State Department verifies identity, so a pre-signed form will not be accepted.
