What is Form DS-64 used for
The form creates an official record that your passport book, passport card, or both are lost or stolen. The State Department flags the document as invalid, which prevents anyone from using the physical passport for travel. Reporting is a separate step from replacement. The DS-64 cancels the old passport; getting a new one requires a full in-person application.
Who files Form DS-64
Anyone whose valid U.S. passport book or card has been lost or stolen files the report. If the missing passport was already expired, no report is needed. Parents or guardians report on behalf of a minor child whose passport is missing.
If you find the passport before reporting it, you can keep using it. After you report it, it is permanently invalid and should be submitted or destroyed as instructed if found.

Provide as much detail as you can; approximate dates and locations are acceptable when exact ones are unknown.

- The missing passport number and approximate issue date, if known
- Whether the book, the card, or both are missing
- The circumstances: when, where, and how the passport was lost or stolen

How to submit it
There are three ways to report. The fastest is online through the State Department's reporting tool, which cancels the passport within about one business day and sends a confirmation email. You can also mail the signed DS-64 with a photocopy of your photo ID, which takes several weeks to process.
If you need a replacement, submit the DS-64 together with Form DS-11 in person at an acceptance facility or passport agency. Replacements after loss or theft cannot use the mail-in renewal form, and cancellation through the in-person route can also take several weeks.
