It applies soon after issuance or when the government printed something incorrectly.

What is Form DS-5504 used for

  • Name change within one year of your passport's issuance, with a certified marriage certificate, divorce decree, or court order
  • Correction of a printing or data error in your current valid passport
  • Replacement of a limited-validity passport, generally within the window stated in the form instructions

Who files Form DS-5504

File it if your passport is valid but shows a data error, if your name changed within one year of the issue date, or if you hold a limited-validity passport, such as one issued abroad in an emergency, and you qualify for a full-validity replacement.

If your name changed more than one year after issuance, use DS-82 if you meet its eligibility rules, or DS-11 in person if you do not. Damaged passports are not corrected with this form; they require a new DS-11 application.

How to fill out Form DS-5504

The form looks like the other passport applications, with the addition of the reason for reissue.

  • Name, date and place of birth, Social Security number, and contact information
  • Your current passport number and issue date
  • The reason you are applying: name change, data correction, or limited passport replacement
  • Product selection and the mailing address for the new passport
  • Sign and date the form after printing

How to submit it

Mail the signed form with your current passport, one new compliant passport photo, and the documents supporting your request: a certified name change document for name changes, or evidence of the error for corrections. Use the address in the form instructions and a trackable mailing method, since your valid passport goes with the package.

The new passport and your supporting documents are returned by mail. If you need urgent service, review the expedited options and current fees on travel.state.gov before mailing.
