What is IMM 5476 used for

The form creates or ends a formal representative relationship on an IRCC file. A representative can be paid or unpaid, but paid representatives must be authorized: lawyers or paralegals who are members of a Canadian law society, Quebec notaries, or regulated Canadian immigration consultants (RCICs) in good standing with the College of Immigration and Citizenship Consultants.

Unpaid representatives, such as a family member, friend, or community organization helping at no charge, can also be appointed with this form. Note the distinction with Form IMM 5475, which only authorizes IRCC to release information to someone without letting them act for you.

Who files IMM 5476

The applicant, sponsor, or other party to an IRCC application files the form, one per person being represented.

• Applicants hiring an RCIC, lawyer, Quebec notary, or law society paralegal to handle a visa, permit, permanent residence, or citizenship matter.
• Applicants appointing an unpaid representative such as a relative or friend.
• Anyone cancelling an existing representative, with or without naming a new one.
• Sponsors in family class applications, who file their own copy if they are also represented.

How to fill out IMM 5476

Complete the form in the Universal PDF editor, then download it for signatures. Read each section heading carefully, since the same form handles both appointing and cancelling.

Both you and your representative must sign and date the relevant declarations; unsigned forms are not accepted.

• Applicant information: your name, date of birth, and details of the application the appointment applies to, including any IRCC file number (UCI) you have.
• Appointment of representative: the representative's name, their organization or membership body, membership or licence number for paid representatives, and full contact details.
• Declaration about compensation: indicate clearly whether the representative is paid or unpaid.
• Cancellation section: complete this part only if you are ending an existing appointment.
• Signatures: you sign to authorize, and the representative signs to accept the appointment.

Filing tips

Submit IMM 5476 with a new application, or send it to IRCC through the webform or the channel named in your program instructions to add or change a representative on a file already in progress. There is no fee to file the form itself.

• IRCC forms are version-pinned; confirm the current version of IMM 5476 on the official page before you submit.
• Verify a paid representative's status before signing: RCICs in the public register of the College of Immigration and Citizenship Consultants, and lawyers or paralegals with their law society.
• Unauthorized paid representation is an offence in Canada, and using a ghost consultant can jeopardize your application.
• Keep a signed copy; if you later change representatives, you will file a new IMM 5476 to update IRCC.
