Form IMM 5475, Authority to Release Personal Information to a Designated Individual, lets you name a person to whom Immigration, Refugees and Citizenship Canada (IRCC) may release information about your immigration or citizenship file. The designated individual can ask about your case status, but cannot act or make decisions on your behalf.

What is IMM 5475 used for

Privacy law prevents IRCC from discussing your file with anyone else unless you authorize it. IMM 5475 is that authorization: it designates one individual, such as a spouse, relative, friend, or member of Parliament's office, to receive information about your application.

The designation is informational only. If you want someone to actually conduct business with IRCC for you, such as responding to requests or submitting documents on your behalf, use Form IMM 5476, Use of a Representative, instead.

Who files IMM 5475

Any applicant with a file before IRCC can designate someone, and each applicant completes their own form.

  • Applicants who want a family member or friend to be able to check case status for them.
  • Applicants abroad who want a contact in Canada to communicate with IRCC about progress.
  • People assisting with a file informally who do not need, or do not qualify for, representative status.
  • Applicants asking their member of Parliament's office to follow up on a delayed file, where the office typically requests this consent.

How to fill out IMM 5475

Sign and date the form, then include it with your application package or send it to IRCC for an application already in progress, following the instructions for your program.

  • Your details: full name, date of birth, and identifiers for your application, such as a UCI or application number if you have one.
  • The application or file the consent applies to.
  • The designated individual: their full name, relationship to you, and contact details.
  • Signature and date; the consent is not valid unsigned.

Filing tips

There is no fee for filing IMM 5475. IRCC forms are version-pinned, so confirm the current version on the official page before you submit, especially if you downloaded a copy some time ago.

  • Designate someone you fully trust, since they will hear details of your immigration history and case.
  • If you want the person to act for you rather than just receive information, file IMM 5476 instead or in addition.
  • You can withdraw the consent later by notifying IRCC in writing.
  • For substantive help with a stalled or refused application, information access alone rarely solves it; a licensed immigration consultant (RCIC) or immigration lawyer can review the file.
