Form ISP-3134 is the Request for Reconsideration of an Old Age Security Decision. If Service Canada refuses your Old Age Security pension or Guaranteed Income Supplement, calculates an amount you believe is wrong, or makes another decision you disagree with, a reconsideration is the first formal step to challenge it. Staff who were not involved in the original decision review your file again along with anything new you provide.

What is Form ISP-3134 used for
The form asks Service Canada to take a second look at a decision made under the Old Age Security program, which covers the OAS pension, the Guaranteed Income Supplement, and the Allowances. It structures your request: which decision you mean, why you disagree, and what new information supports your position.
Reconsideration is a required step before an appeal. If you disagree with the reconsideration decision, your next recourse is an appeal to the Social Security Tribunal of Canada, which has its own process and deadlines described on canada.ca.
Who applies with Form ISP-3134
Anyone who received a written OAS program decision can request reconsideration, whether the decision refused a benefit, set an amount, fixed a start date, or created an overpayment. A representative can prepare the request, but consent needs to be on file, for example through Form ISP-1603, for Service Canada to discuss the case with them.
Timing matters. Requests generally must be made in writing within 90 days of the day you received the decision, and canada.ca explains how to ask for more time if you have missed the window for a good reason.
How to fill out Form ISP-3134
Have the decision letter beside you while you complete the form:
- Enter your Social Insurance Number, name, address, and telephone number.
- Identify the decision you want reconsidered, including the date on the decision letter and the benefit it concerns.
- Explain why you disagree, plainly and specifically, referring to the facts of your situation.
- Attach any new documents that support your case, such as residence records, income corrections, or medical information where relevant.
- Sign and date the request.
How to submit it
Mail the signed form and attachments to the Service Canada office for your province or territory, using the addresses on canada.ca, or deliver the package to a Service Canada office. Keep copies of everything, including the decision letter you are contesting. Service Canada sends the reconsideration outcome in writing, and that letter is what you would take to the Social Security Tribunal if you appeal.
