Form ISP-3025 is the application for the Guaranteed Income Supplement, the non-taxable monthly amount added to the Old Age Security pension for lower-income seniors living in Canada. It is the standalone GIS application, used by people who already receive or have been approved for the OAS pension. The 8-page form collects marital status and income details for you and your spouse or common-law partner.

What is Form ISP-3025 used for

The Guaranteed Income Supplement tops up the OAS pension for pensioners whose income falls below thresholds set each year. ISP-3025 gives Service Canada the information needed to test your income, primarily your marital status and the income you and your spouse or partner reported for the previous year.

The supplement amount depends on your situation, for example whether you are single, married to another pensioner, or married to someone who does not receive OAS. Current thresholds and rates are on canada.ca.

Who applies with Form ISP-3025

Use this form if you receive the Old Age Security pension, live in Canada, and your income, or your combined income as a couple, is within the GIS limits. New OAS applicants do not need it, because Form ISP-3550 applies for OAS and GIS together.

It is also the form to use if you were previously turned down for GIS but your income has dropped, or if you never applied and think you may now qualify. In many cases Service Canada assesses GIS automatically from your tax return, so check your OAS statements or My Service Canada Account before applying.

How to fill out Form ISP-3025

The form is short but the income details matter:

  • Enter your Social Insurance Number, name, date of birth, and address.
  • Report your marital status and, if you have a spouse or common-law partner, their identifying information.
  • Complete the income section for you and your spouse or partner, following the line-by-line instructions on the form.
  • Note any recent retirement or reduction in pension income, since GIS can be based on estimated current-year income after events like retiring.
  • Sign and date the form, and have your spouse or partner sign where the form requires it.

How to submit it

Mail the signed form to the Service Canada office for your province or territory, using the addresses on canada.ca, or drop it off at a Service Canada office. GIS entitlement is reviewed every year, and filing your income tax return on time each year normally renews it without a new application.

Frequently asked questions

Usually not. Once you receive GIS, filing your income tax return by the deadline each year lets Service Canada renew it automatically. You would only reapply if your benefit stopped, for example after a late tax filing or a change in circumstances.
