Form INS5210 is the Request for Reconsideration of an Employment Insurance Decision. If the Canada Employment Insurance Commission denies your EI claim, imposes a penalty, or decides something else about your benefits that you believe is wrong, this 2-page form is how you ask for the decision to be reviewed. An officer not involved in the original decision looks at your file again, together with any new information you provide.

What is Form INS5210 used for
The form starts the formal review of an EI decision. Print the completed form and mail it to one of the addresses printed on the form itself, or take it to a Service Canada office. Send it promptly so it arrives within the 30-day window, and keep a copy along with proof of when you sent it. The Commission communicates its reconsideration decision to you, and that decision is what you would appeal to the Social Security Tribunal if you still disagree.
