Form ISP-1603 is the Consent to Communicate Information to an Authorized Person for Canada Pension Plan and Old Age Security files. Privacy rules stop Service Canada from discussing your benefits with anyone else, even a spouse or adult child, unless you have given written consent. This form records that consent and names the person you trust to speak with Service Canada about your file.

What is Form ISP-1603 used for
The form lets Service Canada exchange information about your CPP or OAS file with a person you designate, such as a family member, friend, accountant, or other representative. The consent is about information only. It does not let the authorized person receive your payments, change where payments go, or make decisions for you, and it is not a power of attorney. Who applies with Form ISP-1603
Any CPP or OAS applicant or beneficiary can file the consent. It is common for seniors who want a family member to handle phone calls, for people managing benefits across languages or time zones, and for anyone whose representative, such as a bookkeeper or lawyer, deals with Service Canada for them.
You choose who to authorize and you can cancel or replace the authorization whenever you like by notifying Service Canada in writing.
How to fill out Form ISP-1603
The form is brief and takes only a few minutes:
- Enter your Social Insurance Number, name, date of birth, and contact information.
- Indicate which programs the consent covers, your Canada Pension Plan file, your Old Age Security file, or both.
- Name the person or organization you are authorizing and provide their address and telephone number.
- Review the scope of the consent so you understand what the authorized person can and cannot do.
- Sign and date the form. Consent is not valid without your signature.
How to submit it
Mail the signed form to the Service Canada office for your province or territory, using the addresses on canada.ca, or drop it off at a Service Canada office. Keep a copy for your records, and tell your authorized person once Service Canada has the consent on file so they can start dealing with the office directly.
Frequently asked questions
No. It only allows Service Canada to share information about your file with the person you name. They cannot redirect payments, apply for benefits for you, or make decisions on your behalf.
No. A power of attorney is a separate legal document with much broader effect. If someone needs to manage your finances or act for you, that requires the appropriate legal arrangement, not this consent form.
The form names a specific authorized person or organization. If you want to change or add representatives, contact Service Canada about filing updated consent.
Notify Service Canada in writing that you are withdrawing the authorization. The consent stays in effect until you cancel it or replace it.
It applies to the Income Security Programs files you select on the form, meaning your Canada Pension Plan and Old Age Security records. It does not cover other government programs.
