What is Form ISP-1151 used for

ISP-1151 applies for the CPP disability pension, a monthly taxable benefit for contributors who can no longer work regularly because of a severe and prolonged disability. The application asks about your medical condition, treatments, work history, education, and daily limitations so adjudicators can assess how your condition affects your capacity to work.

The same application also covers benefits for your dependent children. If you have children under 18, or between 18 and 25 in full-time attendance at school or university, they may qualify for a monthly children's benefit when your disability application is approved. Who applies with Form ISP-1151

You can apply if you are under 65, made enough valid contributions to the Canada Pension Plan, and have a mental or physical disability that is both severe, meaning it regularly stops you from doing any substantially gainful work, and prolonged, meaning it is long-term and of indefinite duration or likely to result in death.

The contribution requirement is generally based on your recent years of work. If you stopped working some time ago, provisions such as the child-rearing provision or a credit split may help you meet it. Someone with authority to act for you, such as a person holding power of attorney, can complete the form on your behalf if you are unable to.

How to fill out Form ISP-1151

Set aside time for this one; it is the most detailed of the CPP applications. Work through it in order:

• Complete the identification section with your Social Insurance Number, date of birth, and contact information.
• Describe your medical condition, your treatments and medications, and the health professionals involved in your care.
• Explain how your condition limits daily activities and why you can no longer work regularly. Be specific and factual.
• List your work history, including your last employer, your duties, and the date you stopped working.
• Add your dependent children so children's benefits can be assessed at the same time.
• Ask your doctor or nurse practitioner to complete the separate medical report, Form ISP-2519.
• Sign the declaration and the consent sections that let Service Canada verify your medical and employment information.

How to submit it

Mail the signed application to the Service Canada office for your province or territory using the addresses in the kit and on canada.ca, or bring it to a Service Canada office. The medical report can be sent with your application or forwarded separately by your practitioner; applying does not have to wait for the medical report if it will follow.

An online application is also available through My Service Canada Account. Whichever route you choose, apply as soon as you believe you qualify, because payments cannot start until an application is received and approved.
