The report moves from identification through clinical detail to prognosis:
- The patient completes the identification section, including their Social Insurance Number, and signs the consent authorizing release of the information.
- The practitioner records the diagnosis and the date the main condition began, along with any other conditions that affect the patient.
- Relevant examination findings, laboratory results, and imaging are summarized, with dates where possible.
- Current and past treatments, medications, and referrals are listed, including how the patient responded.
- The practitioner describes functional limitations, such as restrictions on sitting, standing, lifting, concentration, or attendance, and gives a prognosis.
- The practitioner signs and dates the report and adds their contact and registration details.
How to submit it
The completed report is mailed to the Service Canada office serving the applicant's province or territory, either together with the ISP-1151 application or separately by the practitioner's office. It can also be dropped off at a Service Canada office. Applicants using the online disability application through My Service Canada Account still need the practitioner to provide this medical evidence.
