The request for rollover of whole balance of super benefits between funds (NAT 75359) transfers your entire superannuation balance from one fund to another under the Superannuation Industry (Supervision) Act 1993. It is the paper route for consolidating super or moving to a new fund, including into a self managed super fund, and it is sent to the fund you are leaving rather than to the ATO.

What this form can and cannot do
The form moves the whole balance of one super account to another fund. It cannot transfer only part of a balance, change the fund your employer pays contributions into, open a new super account, transfer benefits when you do not know where your super is, or handle transfers restricted by circumstances such as a superannuation agreement under the Family Law Act 1975.
If you are consolidating several funds, complete a separate rollover form for each transferring fund. To redirect future employer contributions, use the superannuation standard choice form instead.
Who uses it
Fund members moving their balance between large funds, or between a large fund and an SMSF, in either direction. Because super funds must process rollovers through SuperStream, an SMSF involved in the transfer needs an ABN, an electronic service address, and up to date bank account details.
The trustee of the fund you are leaving may ask for additional evidence to confirm your identity, and for SMSF transfers may ask for information about the SMSF bank account to confirm the payment destination. The request cannot be processed until that information is provided.
How to fill out NAT 75359
Complete all mandatory fields, which are marked on the form:
- Personal details: name, date of birth, gender, contact number, and residential address, plus your previous address if the transferring fund may hold an old one.
- Tax file number: optional under the legislation, but there may be tax consequences if you do not provide it.
- FROM fund: fund name, membership or account number, ABN, and unique superannuation identifier for non-SMSF funds.
- TO fund: the same identifiers for the receiving fund, and an answer to whether you are rolling over a super death benefit.
- SMSF transfers: account name, BSB, account number, and electronic service address for the SMSF side of the transfer.
- Authorisation: read the declarations, then print your name, sign, and date the form.
Where to send it
Send the completed and signed form, with any documents your fund requires, to the transferring FROM fund, the one currently holding your money. It is not lodged with the ATO.
Before you sign, you can ask your current fund about exit fees, insurance cover you may lose, and any other effects of the transfer. The authorisation you sign confirms you have obtained that information or decided you do not need it.
Frequently asked questions
No. NAT 75359 is strictly for whole of balance rollovers. For partial transfers, contact your fund about its own partial rollover process.
No. Future employer contributions are directed with the superannuation standard choice form. If you roll over your balance but do not update your choice of fund, contributions may keep flowing to the old fund and reopen it.
