The Superannuation Standard Choice Form (NAT 13080) is how an Australian employee tells their employer which superannuation fund to pay compulsory contributions into. It covers three situations: nominating an existing super account, accepting the employer's default fund, or directing contributions to a self managed super fund (SMSF). Employers also use the form to record their nominated default fund before offering choice to a new starter.

What the standard choice form is used for
Choice of fund is a legal entitlement for most employees, and this form is the standard way to exercise it. Section A records your details and which option you are choosing. Section B captures an existing fund's name, Australian business number (ABN), unique superannuation identifier (USI), and your member account number. Section C is the employer's default fund option, and Section D collects the ABN, electronic service address (ESA), and bank account details for an SMSF.
If you do not return the form, your employer must generally ask the ATO for your stapled super fund, the existing account that follows you between jobs, before falling back to their default fund.
Who uses it
Employees complete it when they start a new job, or at any later point when they want future employer contributions paid to a different fund. Employers must complete the default fund details in Section C before handing the form to an employee, and they keep the completed form for their records.
The form only redirects future employer contributions. It does not move money you already have in super, and it cannot be used to consolidate accounts. For that, see the request for rollover of whole balance between funds.
How to fill it out
Work through the form based on the choice you are making:
- Section A: your full name, employee number if known, and optionally your tax file number. You do not have to provide your TFN, but without it your contributions may be taxed at a higher rate and your fund cannot accept personal contributions.
- Choosing an existing fund: complete Section B with the fund name, ABN, USI, member account number, and your name exactly as it appears on the account, then attach the fund's letter of compliance.
- Choosing the employer default fund: check that your employer has completed their part of Section C, then sign the election.
- Choosing an SMSF: complete Section D with the fund's ABN, electronic service address, and bank account details, and attach evidence from the ATO that the SMSF is regulated, which you can obtain via superfundlookup.gov.au.
- Sign and date the declaration for the section you completed, then return the form to your employer as soon as possible.
Where the form goes
Return the completed form to your employer, not to the ATO or your super fund. The ATO does not collect the information on it; the form simply passes your fund details to your employer so payroll can direct contributions correctly.
Handle the completed PDF carefully, because it can contain your tax file number and account details. Send it through a secure channel your employer nominates rather than an open email thread where practical.
Frequently asked questions
For most employees starting on or after 1 November 2021, the employer must request your stapled super fund details from the ATO and pay contributions there. If the ATO cannot identify a stapled fund, contributions go to the employer's default fund listed in Section C.
No, it is not an offence to withhold it. However, without your TFN your contributions may be taxed at a higher rate, your fund cannot accept personal contributions from you, and it is harder to keep track of your super. Treat the completed form as sensitive either way.
The unique superannuation identifier is on your fund's website or available by contacting the fund. Your member account number appears on your member statement, in your fund's online portal, and in ATO online services via myGov.
No. It only directs where future employer contributions are paid. To transfer an existing balance between funds, use the request for rollover of whole balance of super benefits between funds (NAT 75359) or consolidate through ATO online services.
The SMSF's ABN, an electronic service address so the fund can receive SuperStream messages, the fund's bank account details, and evidence that the SMSF is an ATO regulated fund. Employers can refuse contributions to a fund that cannot accept them electronically.
