Form SA332A is the Services Australia medical report used to assess Carer Payment and Carer Allowance for a person 16 years or over, and it can also support an assessment of Special Disability Trust beneficiary status. It is completed by a treating health professional, not by the carer, and it is based on the Adult Disability Assessment Tool, which measures how much help an adult needs with daily living because of disability or a medical condition.

What is Form SA332A used for

The report gives Services Australia the medical evidence to decide eligibility for Carer Allowance, an income supplement for people providing additional daily care to an adult with disability, a medical condition, or frailty due to age, and Carer Payment, an income support payment for carers whose caring role stops them supporting themselves through substantial paid work. It can also be used when establishing Special Disability Trust beneficiary status.

The professional records the person's conditions, whether daily help is needed with routine personal activities, how long the condition is expected to last, and detailed responses across day to day functions such as mobility, feeding, dressing, and bathing, plus cognitive function and behaviour where relevant.

Who completes Form SA332A

The report must be completed by a health professional currently involved in treating the person being cared for. Accepted professionals are a legally qualified medical practitioner, a registered nurse, a physiotherapist, an occupational therapist, a member of an Aged Care Assessment Team, or an Aboriginal health worker in a geographically remote area.

The carer completes the identifying details and selects what the assessment is for, and the person being cared for, or their nominee, signs the authorisation allowing their medical details to be released to Centrelink.

How to fill out Form SA332A

The form sets out clear steps for the carer before the health professional takes over:

  • Enter the details of the person being cared for, including their date of birth and any Centrelink or Department of Veterans' Affairs reference number.
  • Enter the carer's own details, Customer Reference Number, and contact information.
  • Tick whether the assessment is for Carer Allowance, Carer Payment and Carer Allowance, or Special Disability Trust beneficiary status.
  • Have the person being cared for, or their nominee, sign the authorisation for release of medical details.
  • Book an appointment with the treating health professional and tell the receptionist the report needs to be completed during the consultation.
  • The professional answers the medical questions, notes any information that should not be released directly to the patient, and signs with their qualifications.

How to return it

The completed report can be handed back to the carer to lodge together with the Carer Payment or Carer Allowance claim, or the health professional can send it directly to Centrelink. If the professional has flagged information that could harm the person's wellbeing if released, the report must go directly to Services Australia, Carer Services, PO Box 7805, Canberra BC ACT 2610.
