How to fill out Form PPTC 153

- Personal information: legal name, all other names used, date and place of birth, and contact details
- Recent address and occupation history for the period the form asks about
- Guarantor section: a person who meets the Passport Program criteria and has known you for at least two years completes and signs it
- Two references who have known you for at least two years and are not family members
- Details of your most recent Canadian passport or travel document, if you had one
- Declaration and signature, signed by hand on the printed form
Guarantor and photo requirements
General applications need a guarantor. For applications in Canada, the guarantor is normally the holder of a Canadian passport who has known you for at least two years; the exact criteria are on the official Passport Program pages, and the guarantor signs the form, the back of one photo, and copies of your supporting identity documents where instructed.
You also need two identical passport photos taken by a commercial photographer within the past six months, meeting the Passport Program photo specifications. The photographer's studio name and the date should appear on the back.
How to submit it
In Canada, you can submit the application in person at a Service Canada Centre that offers passport services or at a passport office, or mail it to the address in the form instructions. Applying in person is the usual choice when you need the passport quickly or want your original documents handled over the counter.
