What is Form IMM 5009 used for

A verification of status is an official statement of the information IRCC holds about your immigration history, such as when you became a permanent resident or the details of a temporary stay. It is frequently needed for citizenship applications, pension and benefit claims, sponsorships, and legal matters where the original document is missing.

The replacement stream reissues immigration documents themselves. Older immigrants often use it to replace a lost Record of Landing, which predates the PR card and remains important evidence for things like proving residence history or applying for benefits.

Who uses IMM 5009

The person the record concerns usually applies, but the form also accommodates requests connected to another person's record in defined situations described in the instructions, with appropriate consent or proof of authority.

• Permanent residents or former permanent residents who lost their landing papers or confirmation of permanent residence.
• People who need official confirmation of their immigration status or history for another government process.
• Representatives applying on someone's behalf with the required authorization documents.

How to fill out IMM 5009

Give IRCC everything you can to locate the record: the more identifiers you provide, the faster the search. Records from earlier decades can be indexed under different name spellings, so note any variations you have used.

• Choose whether you are requesting a verification of status or the replacement of a specific document, and identify which document.
• Provide your full name at the time of the original document, plus any later name changes with supporting documents.
• Include your date of birth, country of birth, and the approximate date and port of entry or office that issued the document, if known.
• Attach copies of identity documents as the checklist requires, sign the form, and check the current fee on the IRCC fee list before submitting.

Processing and practical notes

Searches of historical records can take time, especially for landings from before electronic records. Apply well ahead of any deadline that depends on the document, such as a citizenship application or pension claim.
