SGLV 8721, the VGLI Beneficiary Designation, is the form Veterans' Group Life Insurance policyholders use to name or change the beneficiaries of their coverage. The designation on file with the Office of Servicemembers' Group Life Insurance (OSGLI) controls who is paid, regardless of what a will says.

What is SGLV 8721 used for
The form records who receives VGLI proceeds and in what shares. Because VGLI is a post-service policy administered by OSGLI, changes go directly to that office rather than through a military personnel office.
The most recent valid designation on file at the time of death is the one that is paid. Filing a new SGLV 8721 replaces earlier designations, so the form is also how you remove a former beneficiary.
Who uses SGLV 8721
Any VGLI policyholder can file the form at any time. Common triggers are marriage, divorce, the birth of a child, the death of a previously named beneficiary, or simply a change of mind.
Life events do not update the designation automatically. A divorce, for example, does not remove a former spouse from a VGLI designation; only a new designation does that.
How to fill out SGLV 8721
The form is brief, and precision is what matters most:
- Policyholder information: name, Social Security number, and contact details
- Beneficiary details: each principal beneficiary's full name, address, relationship, and share of the proceeds
- Contingent beneficiaries: who is paid if a principal beneficiary dies before you
- Shares: make the percentages add up to 100 percent for each level of beneficiary
- Signature and date: sign the printed form before mailing it

After the change is processed, confirm it through the portal or by contacting OSGLI, and keep a dated copy of the form with your records.
