- Provide two identical passport photos that meet the current standards, and write your full name on the back of one of them. The lodgement officer places one face up and one face down.
- Sign inside the white signature box using dark ink, keeping the signature within the lines. If you are unable to sign, mark the box provided instead.
- Print your family name, given names and date of birth in block letters using dark ink.
- Take the completed form, your photos and your original documents to lodgement as directed by the Passport Office, where the official use section is completed.
Getting it right the second time
Because this form exists to fix a rejected photo or signature, it is worth double checking the requirements before lodging it. Photos must be identical prints of the same recent image, correctly sized, with a plain light background and a neutral expression, and signatures must stay inside the box and be made in dark ink.
Making a false or misleading statement in connection with a passport application is a serious offence, and the form carries that warning. If anything about the request is unclear, contact the Passport Office before lodging rather than guessing.
