IRS Form 941-X, the Adjusted Employer's Quarterly Federal Tax Return or Claim for Refund, is the form employers use to fix mistakes on a Form 941 they already filed. What is Form 941-X used for
Form 941-X exists to correct a Form 941 that has already been submitted to the IRS. Employers use it when they discover that reported wages, tips, withheld income tax, or Social Security and Medicare amounts were wrong for a particular quarter.
The form supports two processes. The adjustment process applies underreported or overreported amounts against your current filings, while the claim process requests a refund or abatement of overreported tax. You choose one of the two on the form itself.
Who needs to file Form 941-X
Any employer that filed a Form 941 with an error needs to file Form 941-X to correct it. This includes corrections that increase the tax owed as well as corrections that reduce it.
A separate Form 941-X is required for each quarter being corrected. You file it on its own rather than attaching it to a regular Form 941.
How to fill out Form 941-X
The form walks through the correction step by step. Work from your originally filed Form 941 and your corrected payroll records side by side.
- Enter your employer identification number (EIN), name, and address exactly as they appeared on the original Form 941.
- Check the box for the quarter and enter the calendar year you are correcting, along with the date you discovered the errors.
- In Part 1, choose either the adjustment process or the claim process for the corrections on this form.
- In Part 2, complete the certifications that apply, including those about repaying or reimbursing employees for overcollected amounts.
- In Part 3, enter the corrected amount, the amount originally reported, and the difference for each line you are changing.
- In Part 4, explain in detail why you are making each correction, then sign and date the form.
Where to send it and deadlines
Form 941-X is generally subject to a period of limitations. In most cases you must file it within three years of the date the original Form 941 was filed or two years from the date you paid the tax, whichever is later; confirm the details in the current IRS instructions.
The IRS began accepting electronically filed amended employment tax returns, including Form 941-X, in 2024. If you file on paper, the mailing address depends on your location, so check the address table in the official instructions before sending it.
