Form 47A, Details of child or other dependent family member aged 18 years or over, is a supplementary form used with Australian migration visa applications. It must be completed for each member of the family unit aged 18 or over, whether or not that person is migrating, so the Department of Home Affairs has full details of everyone in the applicant's family unit.
The form collects the family member's personal details, identity documents and address history, and it contains an Australian values statement that the family member signs if they are applying to migrate. What Form 47A is used for
Australian migration applications ask for information about every member of the applicant's family unit, such as a partner or children, even those not intending to migrate. Form 47A is how the details of adult dependants are provided: one form for each family member aged 18 or over, whether they are included in the visa application or staying behind.
Who counts as a member of the family unit for migration purposes is defined in the Department's information form 1496i, Including family members in your application, which is worth reading before completing Form 47A. Adult children, for example, generally need to be dependent on the main applicant to be included.
Who completes it
The form is completed for the dependent family member aged 18 or over, and that person signs it. If the family member is applying to migrate, they also sign the Australian values statement inside the form, confirming they have read or had explained to them information about Australian society and values and will conduct themselves accordingly and obey Australian laws.
- Complete one Form 47A for each family unit member aged 18 or over, migrating or not.
- Family members not applying to migrate do not need to sign the values statement.
- Members being added to an application lodged before 15 October 2007 may not need to sign the statement; the form suggests speaking to the case officer if there are concerns.
- The Department encourages applicants to read the Life in Australia booklet, available from the Home Affairs website in many languages, before signing.
How to fill out Form 47A
Complete the form in English and answer every question that applies to the family member. If there is not enough space for an answer, continue under the Additional information part at the end of the form, quoting the question number.
- Personal details: the family member's names, date and place of birth, citizenship and relationship to the main applicant.
- Identity documents: passport or travel document details and any national identity numbers.
- Previous addresses: every address where the family member has lived for 12 months or more in the last 10 years, plus all addresses in Australia for any period, including visits. This question must be answered even if they have lived at only one address.
- Additional information: overflow answers keyed to their question numbers.
- Signatures: the family member signs the form, and signs the Australian values statement if they are applying to migrate.
How to lodge Form 47A
Form 47A is not lodged on its own. Include the completed form with the main visa application it supports: for online applications that generally means uploading it to the application in ImmiAccount, and for paper applications it is attached to the application form, following the lodgement instructions for the specific visa subclass.
Keep a copy for your records. Home Affairs forms are version dated, so check the official website for the current version of Form 47A, and consider advice from a registered migration agent if you are unsure whether an adult relative qualifies as a member of the family unit.
