The medical document is the foundation of Canada's medical cannabis system. It is completed and signed by an authorized health care practitioner and authorizes a patient's use of cannabis for medical purposes, stating the daily quantity and the period of use. Health Canada publishes this sample format for practitioners under the Cannabis Regulations.

What the medical document is used for
Under the Cannabis Regulations, a medical document is what allows a patient to access cannabis for medical purposes through the regulated channels. The completed document is used in one of two ways:
- Registering with a federally licensed seller, which lets the patient order cannabis directly from that seller
- Registering with Health Canada to produce a limited amount of cannabis personally or through a designated person
Who completes the medical document
An authorized health care practitioner, such as a physician or nurse practitioner where permitted, completes and signs the document following an assessment of the patient. The patient does not fill in the medical content themselves.
This page explains the paperwork, not the medicine. Whether cannabis is appropriate for a patient is a clinical decision made by the practitioner, and provincial and territorial rules also affect who may sign. Universal PDF is not affiliated with Health Canada.
What the form asks for
The sample format published by Health Canada covers the elements the regulations require:
- Practitioner details: name, profession, licence number, the province or territory of practice, and business contact information
- Patient details: the patient's name and date of birth
- Authorization details: the daily quantity of dried cannabis in grams, or its equivalent, and the period of use
- The practitioner's signature and the date signed
How the document is used after signing
The signed original goes to whichever registration route the patient chooses: a federally licensed seller, or Health Canada for a personal or designated production registration. Sellers and Health Canada verify the document, and the details on any registration application must match it.
The period of use on the document limits how long the resulting registration lasts, so patients need a new medical document when it expires to keep their registration active.
