DD Form 2866, the Retiree Change of Address/State Tax Withholding Request, is the DFAS form military retirees use for two routine account updates: changing the mailing address on a retired pay account and starting, stopping, or changing state income tax withholding from retired pay. How to fill out DD Form 2866
