DD Form 1351-2, the Travel Voucher or Subvoucher, is how service members, DoD civilians, and eligible dependents claim reimbursement for official travel. It covers temporary duty (TDY) trips and permanent change of station (PCS) moves, and it is the document finance offices use to compute what you are owed.

What is DD Form 1351-2 used for
The voucher settles your travel claim after the trip. It reports where you went, how you traveled, and what you spent, and the servicing finance office uses it together with your orders to calculate per diem and expense reimbursements under the Joint Travel Regulations.
The form is used when a claim is not processed through the Defense Travel System, which handles many routine TDY claims electronically. Typical paper or PDF voucher situations include:
- PCS travel claims for members and their dependents
- TDY travel that was not booked or settled in DTS
- Supplemental claims to correct or add to an earlier settlement
Who files DD Form 1351-2
The traveler files the voucher: military members after PCS or eligible TDY travel, DoD civilian employees for covered official travel, and, in some cases, family members traveling on the member's orders. Claims are generally expected promptly after travel is completed, commonly within five working days of returning.
For most TDY claims a supervisor or other reviewing official also signs the voucher before it goes to finance.
How to fill out DD Form 1351-2
Have your orders and receipts in front of you, then work through the blocks:
- Payment and identification information, including your electronic funds transfer details and daytime contact
- The travel order or authorization number from your orders
- The itinerary: every departure and arrival point with dates, and the means of travel for each leg
- Reimbursable expenses, itemized, such as lodging, transportation, and other authorized costs
- Receipts attached for lodging and for any expense at or above the receipt threshold in the Joint Travel Regulations
- Your signature and date, plus the reviewer or approving official signature where required
Where to send it
Where the voucher goes depends on your service and the type of travel. Many PCS claims are submitted to the Defense Finance and Accounting Service, including through its online SmartVoucher submission tools, while other claims go to your local or servicing finance office. Your orders or the losing and gaining installations' finance pages will state the route for your claim.
Keep copies of the voucher, orders, and every receipt until the claim is fully settled, and verify the current form edition with your finance office or the official DoD forms site.
Frequently asked questions
If your TDY was arranged and authorized in the Defense Travel System, the claim is normally settled there too. The 1351-2 is used when a claim runs outside DTS, which includes most PCS travel and situations where DTS was not available.
Lodging receipts are always required, and other expenses need receipts once they reach the threshold set in the Joint Travel Regulations. When in doubt, attach the receipt, since missing documentation is a top reason vouchers bounce back.
It is the continuation sheet for the voucher. If your itinerary or expense list does not fit on the main form, carry the overflow onto the 1351-2C and submit the two together.
File promptly once travel is complete; the common standard is within five working days of returning. PCS claims should be filed after arrival at the new duty station following your service's procedures.
