The change of details for individuals form (NAT 2817) updates the personal information the Australian Taxation Office holds about you: your name, postal and home addresses, and phone and email contacts. It can also nominate another person as your representative, an authorised contact permitted to deal with the ATO and receive information about your taxation affairs on your behalf.

What you can change with this form

Section B covers changes to your legal name, postal address, and home address. A name change must be supported by a certified copy of an identity document, such as an Australian full birth certificate, an Australian change of name certificate, or an Australian marriage certificate, with foreign document alternatives available for foreign residents, including a foreign passport, foreign birth or marriage certificate, or foreign drivers licence.

If you operate as a sole trader, the form can push the same name, address, and contact updates through to the Australian Business Register, so your business records stay aligned.

Nominating a representative

Section D lets you authorise another person to deal with the ATO for you, either across all of your taxation affairs or limited to specific matters you list. Only you can sign the declaration when nominating a representative, and the nomination continues indefinitely until you tell the ATO it no longer applies.

The form cannot be used to appoint a professional such as a registered tax agent, BAS agent, barrister, or solicitor as your representative; those appointments run through their own channels. Nominating someone does not cancel any earlier nomination, so review who already has authority on your record.

How to fill out NAT 2817

Sections A, C, and E are compulsory:

  • Section A: your TFN, plus your name, postal address, home address, and date of birth as they were when you last dealt with the ATO, so the ATO can find your record.
  • Section B: only the details that have changed, with a certified supporting document for a name change.
  • Section C: your current phone numbers and email address, and whether you want them updated on ATO records and, for sole traders, the Australian Business Register.
  • Section D: your representative's details and the scope of their authority, if you are nominating one.
  • Section E: the declaration. If you nominated a representative, only you can sign; otherwise you or a person already authorised on your record can sign.

Where to send it

Post the signed form and any certified supporting documents to Australian Taxation Office, PO Box 3373, Penrith NSW 2740, and keep a copy for your records. The ATO aims to process it within 28 days of receiving everything needed.

Because the form carries your TFN and identity documents, send it only by the lodgment channel and store your copy securely. For simple updates, changing your details in ATO online services through myGov avoids mailing identity documents altogether.
